The public screen and official guidance show the field labels and preparation items. They do not expose readable text-field regexes, character limits or official validation errors. This guide invents none and mounts no local validator.
Section 1: Route and eligibility
1. Visa route. Confirm visa-on-arrival eligibility first. Indian citizens need no visa; the current restricted list must use an advance route.
2. Entry point. Choose the airport or land office where you will actually enter and receive the visa.
3. Passport country. Select the country that issued the travel passport, not the country of residence. Gratis treatment follows the eligible category.
Section 2: Passport and identity
4. Passport number. Use the passport that will be presented at the immigration desk.
5. Date of birth. The live first screen says Date of Birth (AD) and uses day, month and year choices. Enter the Gregorian date from the passport.
6. Full name. Copy the complete passport name. Do not shorten it to a booking nickname.
7. Passport validity. The Nepal Embassy in Beijing says more than 6 months is required for processing.
8. Digital photo. Prepare a recent passport-size digital photograph. No verified public pixel or file-size limit is claimed here.
9. Addresses. Enter the detailed permanent address and the full hotel or local address in Nepal.
10. Contact and duration. Give a working phone number and email. Choose 15, 30 or 90 days before payment.
After submission
Save or print the barcode receipt before closing the page. It remains valid for 15 days. Present the form, payment receipt and passport at the immigration desk; the officer issues the visa there.
