On arrival: at the port of first arrival, aboard a conveyance with an assigned CBP officer, or at a preclearance office in a foreign country (19 CFR 148.11). No standalone departure goods declaration form is inferred from these provisions.
Who it applies to, exceptions and limits (full text)
Under 19 CFR 148.11, all articles brought into the United States by an individual must be declared to a CBP officer at the port of first arrival, on an assigned conveyance en route, or at a foreign preclearance office. Under 148.12, returning residents and nonresidents may make an oral declaration when its conditions are met, but a written declaration may be required generally, by traffic type, or in particular cases, and identifying information on CBP Form 6059-B may still be required. Under 148.13, unless an oral declaration is accepted, the declaration must be in writing on Customs Form 6059-B. This procedure is stated for the 19 CFR 101.1 customs territory, which includes only the States, DC and Puerto Rico; Virgin Islands ports have their own customs laws (48 U.S.C. 1406(i)) and lie outside that territory, so do not assume this form and process automatically cover arrival in the USVI or movement between domestic territories.

Travelers carrying the currency or monetary instruments file FinCEN Form 105 at the time of entry into or departure from the United States with the Customs officer in charge at any Customs port of entry or departure.
Who it applies to, exceptions and limits (full text)
Each person who physically transports, attempts to transport, or causes the transportation of currency or other monetary instruments in an aggregate amount strictly exceeding US$10,000 at one time, from the United States to any place outside the United States or into the United States from any place outside, must report it on FinCEN Form 105 (31 U.S.C. 5316; 31 CFR 1010.340). Exactly US$10,000 does not trigger the report. "United States" here is the Title 31 definition (31 CFR 1010.100(hhh)), which includes the Territories and Insular Possessions; paragraph (zz) expressly lists Puerto Rico and the U.S. Virgin Islands. Mainland–USVI travel is therefore not, by itself, a reportable movement to or from outside the United States merely because USVI lies outside the 19 CFR 101.1 customs territory. "At one time" under 1010.100(b) aggregates what a person transports alone, jointly, or on behalf of others on one calendar day (or multiple days if to evade reporting); independently held money of separate family members is not automatically pooled. Ordinary bank transfers not involving physical transport are outside this form's requirement.
